I don't know any vanilla method for stopping doctors from feeding animals first food they see, short of zoning doctors away from food. That way, both doctors and their patients will only have access to tasty hay and kibble. I ended up using mods that allow food restrictions for animals.
My best approach was to build the barn in the middle of the pen, then set up animal sleeping spots (which you actually assign) and set up a campfire for heat. Don't forget to build the cloth screen versus a door. I also have a storage spot for the kibble. Forbid hay on every stockpile you have, allow it only for the shelves. Animals eating on their own, if they have access to the food. Actually I always use shelves in pen to feed animals and keep stock outside.My other animals like chickens and alpaca are kept in a mid-sized barn that has a room with sleeping spots and a large room that has some hay and a large rose garden for them to munch on.
